1. I/We acknowledge that I/we have been provided with appropriate information

about the microchip implanting process and AVID PetTrac Recovery

System.

2. I/We, as the new owner of the dog described, acknowledge that the dog has

been implanted with a microchip for identification purposes by Willow Creek

Kennel.

applicable microchip scanner prior and after insertion and has been verified to
